This fix should soon be available as a customer beta and is planned for inclusion in the next customer release. Customer beta builds are announced here and you can participate by following these instructions. If you still experience this problem once you are using that build version (or later) please reply to this thread including @Serif Info Bot to notify us. Quote

It's a fresh install only couple of weeks old and I've started seeing these Tool shortcut duplicates. It is fixed when I wipe it clean with the Serif defaults but come back as soon as I load my custom shortcuts from a file. Any ideas? Juhani Posted July 1, 2024 Posted July 1, 2024 So I wiped all the shortcuts by choosing the Serif defaults, restarted Designer, inserted them again one by one, restarted Designer again, and same stuff, the Tool shortcut lines are duplicated AGAIN. So it cannot be my file, it's definitely this version of the app. Perhaps Affinity does not like some of my shortcuts, because as I was inserting them i did check into the Tools section constantly and they were all fine, no duplicates. But as soon as got them all in and restarted the app the duplicates came immediately back. Now it's the same thing as already described, I can wipe them and the duplicates are fixed, but if I load my own file they come back. I can confirm that the originally logged issue (AF-63) is marked as closed with our development team, as this bug was specific to showing duplicated keyboard entries without the need to edit or add custom shortcuts and this is still true, therefore the issue remains closed. However there is a separately logged bug, whereby editing or adding custom shortcuts to any 'Tool' on macOS (including Tool 'cycle' shortcuts) will display duplicate Shortcut entries after restarting the Affinity app. This appears to be the issue reported above, therefore I will be sure to bump and update this specific development report (AF-1922) for you all now.
