Templates for book design?

Are there any templates for paperback-sized book design in Publisher? I want to convert a document in Word (about 90,000 words) into a format that can be read in e-readers (PDF until Publisher gets an epub export...).

I'd also like to get some templates for books using other standard sizes.

Or... does anyone have suggestions for creating ebooks in the popular file formats using Publisher?

Thanks, but I have been doing layout and book/magazine/newspaper design since the 1980s. What I was hoping for were actual templates I could tweak rather than advice on how to create one from scratch. But I will read his page and see what I can use of it.

Just FYI, I found a few free inDesign templates online. I downloaded one and opened it in AP2 and it seems to have worked. It wasn't the layout I wanted, but that gives me hope. I had in mind something that has the look of Bringhurst's book on typography or maybe Alpha to Omega by Humez.

This can be a fairly variable layout, with mainly only page margins and marginalia columns in common throughout the book...

...which would make it difficult for a “generic template” and may require a lot of custom styling + editing. For the marginal columns APub's "Sidenote" and "Pinning" features can be useful, for example:
