Expand stroke problems

This sort of thing is sometimes caused by coincident nodes (doubles) or nodes very very close to each other. Take a look at the curve while you have the curve tool selected and zoom in to see if there are two nodes very very close together, try moving the nodes with the node tool to check for coincident nodes.
