Fixed it~!
I just had to re-apply the Master to the page for it to show through and export properly. Oops.
I apologise for the wasted topic! If any mods want to delete this (I don't see an option to do this myself?) then please do.

Update, I got around this by making a group of the image and brown pantone (image in erase mode), then putting the grey pantone (rectangle filled with the pantone grey) layer to the back.

Have you tried using the blend mode Erase?
Duplicate the embedded layer and change the blend mode to erase, the problem with this is it creates a bit of a halo. see image below.

One option is to make the book cover in Aff Designer - you can have different sizes of artboards and then is possible open this document in Publisher, but you can't convert artboards to spread.

You cannot have pages of different sizes within a single spread. Nor can you have more than 2 pages in a spread.
At this point you will need to use separate pages, or you will need to use one page with a set of rectangles on it as you showed.

Yes it is possible, by using sections.
If you Right Click on a page in the Pages panel, you can start a new section.

Once you have made a new section you can then Right Click on the page in your section that you want to be the cover page and this time select Spread properties. In the dialogue box that opens you can select Custom size and then enter the size that your book cover needs to be. When you've done that you will now have different size pages. You will then need to make sure all of your book pages are in a new section that starts after your cover.
