It does not remove any blemishes or It does not do any effect.

 

I follow the steps on this video. I’m at 4:04

Nothing as far as I can tell is wrong from your contextual tool settings, If you could provide a screen recording demonstrating the issue we may be able to advise further.

I found the problem.

 

I have to choose the setting 

 

Context Toolbar - Source - Current Layer and Below (Not Current Layer)
