Hello - why can't I make custom sizing for my book cover spread?

When I change the width to incorporate the spine size, the height (standard 210mm) changes too. It's driving me crazy

You most likely have the two dimensions linked. There is a little graphic (of a chain) between or next to the Height and Width number boxes. Click on the little graphic to either lock or unlock the aspect ratio function.

I have never mastered color management, period, so I cannot help with that.

FWIW, you will find a chain icon in a few other places in the UI where two fields can be linked, sometimes vertical like here in the Transform panel:
