I posted about this in the Feedback for V2 - as no other option seemed to be available until I found this tag.

The Index Function is NOT working in V2.  Sorry for repeating but I don't want this BUG  to get lost in the plethora of general feedback.

A large book file upgraded from V1 to V2 - no new Insert Marker and no Index Update - all greyed out.

This does not happen with a new file in V2

However, with a 288-page book that is already fully indexed, this is a serious BUG.   (Actually my experience with several books is that the Afpub Index function is very buggy).

I can't duplicate this problem, my book's index is working perfectly in v2 without any changes from v1.

The state of the Index panel in your screenshot is what you would see if the text cursor is in the index. I realize the cursor is in a simple frame with with just the word "test" but something is messed up with your document for this to happen. I know you've had problems with the index in the past but I think it's happened again.

You might try deleting the index, which hurts nothing since it can be reinserted at any time, and then seeing whether the Insert Index button is re-enabled and works. If it isn't re-enabled, your index was just plain text and your actual index object was in that little test object.

Thanks.  I'll test when I have this book done and published. I've wasted most of today trying to get this work.

I've deleted and re-inserted the Index and the Table of Contents in this book at least 20 times and the same with it's companion.  New Text Frames every time.
