When you say 'Old' - How old?  Is it the same document opening each time? Whatever the reason, if you want your current working document to open each time then check this little box in preferences - the very first preference listed.

Microsoft - Like entering your home and opening the stainless steel kitchen door, with a Popup: 'Do you really want to open this door'? Then looking for the dishwasher and finding it stored in the living room where you have to download a water supply from the app store, then you have to buy microsoft compliant soap, remove the carpet only to be told that it is glued to the floor.. Don't forget to make multiple copies of your front door key and post them to all who demand access to all the doors inside your home including the windows and outside shed.

Apple - Like entering your home and opening the oak framed Kitchen door and finding the dishwasher right in front you ready to be switched on, soap supplied, and water that comes through a water softener.  Ah the front door key is yours and it only needs to open the front door.

Publisher should not open any file automatically unless you have the “Reopen document on startup” option checked as shown above.
If you do not have this option checked then I suspect that you might be opening Publisher by double-clicking on a document icon rather than the application icon. (This might explain why you keep opening the same outdated file.)
If this is not so then you might need to tell (show) us exactly how you are opening Publisher.

Let me solve my own observation after exploring some thing some more (new iMac (OS/X 11.2) to me, so not been through all of the preferences).  

iMac -> System Preferences -> General.  Bottom grouping of options.  There is a "Close windows when quitting an app" option. 

If this option is checked, The Affinity Publisher -> Preferences -> General -> "Reopen Document(s) on startup" will not work. 

Uncheck the iMac systems preferences box noted above and then AP's "Reopen Document... " option will work.

Hope this helps others besides m....
