If you have a keyboard, tap to create the first node, then hold Shift and tap to create the second node.

Or, from the Help for the Pen Tool (https://affinity.help/designeripad/en-US.lproj/pages/Tools/tools_pen.html) you can hold two fingers on the screen while you tap for the second node, and it will constrain the line to a multiple of 45 degrees, so you can be somewhat inaccurate and still get horizontal or vertical.
