I am using v2.04, all a sudden my adjustment layer is not working because Affinity Photo is adding a mask to each layer I create.  How do I turn it off?  I can't work it out.

Most likely you have an selection active while creating an adjustment layer.

try „deselect all“ before adding adjustments.

Adjustment layers are mask layers in their own right, normally you will not add a mask layer but as NOTMYFAULT says if you have even the smallest area selected a mask will automatically be added. 
Another way is if you have a shortcut set to add a mask you may be accidently pressing the key combo to do that?
