The info text for the Export bookmark option implies that it is only a pass-through for Publisher bookmarks. Ideally bookmark should be an option for a text style so a TOC bookmark can be created easily. Quote

You should be able to. Just remember that you need to have Paragraph Styles for the headings or whatever you want in the ToC for the ToC to use. I would think you might need to do the Export from Publisher and not Designer, but I could be wrong about that, I frequently am wrong. You could also use the Hyperlinks in Publisher instead of a ToC.
