The adjustments/filters built-in masks have some data (black lines) that's causing what you see in the image. Press and hold alt and click the adjustment/filter's thumbnails in the Layers panel to see their alpha as a greyscale image (and the aforementioned black lines). To clear them up select each adjustment/filter and fill them with white. There's a few ways to do this, for example, select each adjustment/filter int he Layers panel, go to the Channels panel, right click the corresponding adjustment's alpha channel and select Fill.
