Welcome to Affinity Forums :)

The process is similar in Affinity Photo. Usually you download/get a template from the service/company you want to use to print the shirt and import it to Affinity Photo (to use as a guide). Then create your composition according to the template guidelines and their specifications - they usually have instructions/recommendations to follow and submit the resulting file to them. Here's an example from Printful.
