Color Chooser - colors are wrong

I am not sure what I have done, but colors are no longer displaying correctly.  When I enter the hex code, the color should be correct.  It is actually lighter than it should be.  in addition, the color chooser is not displaying correctly, with the top half appearing to have a white gradient over it,  This is the second time this has happened, so I am sure i am changing a setting somewhere, I just do not know what it is.

Your document is set up using the wrong colour profile (scrgb virtual device model profile).
 
When you open a new document, make sure the colour profile is set to 'sRGB IEC61966-2.1' (or 'Adobe RGB 1998' to use aRGB).

For the two documents that you have already created with this particular colour profile, go to Document > Assign ICC Profile (Note:  Not 'Convert ICC Profile' in this particular case) > Select 'sRGB IEC61966-2.1' from the list > then press the 'Assign' button.

The whitish part of the Color Chooser window is not a gradient. It just shows the color values you get if you click in that area. So for example, anywhere along the top edge will set the Lightness to 100%, producing white, the same as R, G, & B values of 255 for an 8 bit color space.
