When setting up a paragraph style decoration with a side element the alignment settings on the stroke do not change the alignment as expected (nor at all).

Here are some examples. This is what the stroke placement is like with the first alignment setting, the second setting, and the third. All appear identical. I hasve used a thick line to demonstrate.

@emccanno, in this instance you would use the indent settings just above the stroke setting in the Decoration Section rather than the Stroke Align setting in the stroke panel, so you are basically offsetting the decoration from the text frame...
