I am having problems with the TOC. I have a main item which is chapter header, Introduction, and a sub header, Test sub title, for the TOC. The problem is that the sub header comes before the chapter header. I have attached 2 screen shots.

Can anyone give me a suggestion as to what I am doing wrong?

The order of entries in the TOC, when both items are on the same Spread, is determined by their order in the Layers panel, from bottom (first added) to top (last added).

It sounds like you created the Text Frame for "Introduction" after the one for "Test subtitle", which makes "Introduction"  logically (in layer order) below/after the subtitle.

Just drag it lower in the layer stack and then update the TOC.

:D Walt, you've saved my day.  

I had the problem of none existing Heading1 showing up in the TOC. 

Having a good look at the layers panel solved all weird problems.
