Welcome to Affinity Forums :)

That's because the text is a vector object (what you see in Outline view mode is it's geometry) and the image is just a bunch of pixels (you only see the rectangle corresponding to its boundaries in Outline view mode). Those are different types of data. Affinity is able to work with both at the same time.

 

The process to create lines and shapes (geometry) from images is called tracing. You can do it manually, drawing over the image with the Pen Tool, or use an automatic tracing function (not available in Affinity Designer). There's third party software able to do this automatically (transform an image - pixels - into vector data) that you can then import into Affinity.
