As you say it happens sometimes, do you have any idea what you are doing when specifically when it does? We would need to know what triggers the issue to be able to reproduce it. When you see it happening next time would you be able to upload a video of what you are currently doing so we can reproduce the steps? I haven't been able to reproduce this on my iMac so will see if any of my colleagues that are using external monitors can. Can I ask what monitor you are using, what macOS you use and what version of Publisher you are using in case they ask for more details?
