In the past I was able to group multiple groups together and they would stay in their individuals groups, but bow without one larger group. Since the update, I am not able to do this  anymore. When I do, it removes the elements from their individual groups and just makes them into one large group. Which causes more work. I haven't seen an option anywhere related to this, but after years of it doing one things, this doesn't work for me. When I draw sub-assemblies of a larger object, I need them to stay grouped together so they can be changed or re-used other places, but be grouped into the larger object at the same time. I used to be able to do that without any issues, now I can't do it at all.

Is it just me, or has there been a change in how designer acts in the most basic ways?  As simple as draw a square, now it deselects what I just drew, instead of keeping it selected. I feel like I am starting over trying to learn a completely new way to work.  All of the muscle memory built over the past few years doesn't work anymore. Since the update, I have wasted hours having to just stop and redo things because the changes I made weren't applied to anything.

I've tried to read through posts and emails and guides and nothing I have found so far as said anything. I am just missing something, or am I the only person with this issue?

You can rectify this by checking (ticking) the “Keep Selected” setting on the Context Toolbar while the tool is selected.
