Does Affinity photo have anything like Smart Objects in Photoshop?

Yes, you can use embedded documents to achieve similar results in Affinity apps. Insert (drag) an .afphoto or .afdesign document into an existing document, then double-click it on canvas to open it in a new document tab (notice this tab is labelled embedded). Any changes you make to it will be automatically/immediately reflected in the original document where it was embedded. Check those videos for examples both in Photo and Designer:
