Okay, found out answer to my own request.

You can rotate the canvas in sidecar. Turn on the sidebar and you can hold down the CMD button while either scrolling with mouse or rotating with fingers, no longer needing to reach over to keyboard. (I use AD in "separated" window mode and just send the canvas to the iPad while my panels all line the bottom of my Mac's display.)

Though this latter case is kind of iffy because Apple has some real issues still with sidecar, like not recognizing gestures too well. And their constant disconnects, which I fix by unplugging lightning cord and then plugging it back in. (Yes, I only use an iPad Air 3.) Apple really needs to fix this stuff.

You then just need to find the right brush type because of sidecar's somewhat lagginess.

Case you're wondering why I don't use AD for iPad, it's because that's too clunky. I prefer Procreate, by far, but now I need to do vector-based 'sketching."

I don't have an iPad attached so it's just a guessing: Maybe these Preferences > Tools do influence the behavior with sidecar, too:

Yes, they do, and I have that on, but my big breakthrough was opening the side bar and realizing I could use the CMD button on the iPad, meaning my left hand didn't have to leave the screen. And now that I figured out the two-figure twirl works (sometimes, but I think that's an Apple issue), my right hand doesn't have to leave the screen either.
