Very possible! There are probably numerous ways to accomplish this, but here's the process that comes to mind first...

1) My first layer is a photo called "Red Flower."

2) On top of that layer, I added a photo of Keira Knightley. I used the "Place..." command, and then rasterized the layer.

3) I added two adjustments to the Keira layer, only. The first was a Black and White adjustment. The second was a Posterize adjustment. Using the Posterize adjustment (set at 1 level) allowed me to make the Keira level black and white, without shades of grey, like you specified. I thought this would be a bit more elegant than the "Levels" process described in the web site you linked.

4) I used the Flood Fill to fill the second layer with white (so that the mask would compute better).

5) With the Keira layer selected, I chose "Rasterize to Mask" from the Layer menu. I dragged the resulting mask into the Red Flower layer, and put a Black fill layer underneath everything so that the masked photo of Keira had a way to show up nicely.

Unless I've overlooked something (which is entirely possible!) you can collapse those two adjustments into one by replacing them with a Threshold adjustment.
