Skipping Pages after Table of Contents

Has anyone experienced blank pages after a table of contents and not been able to remove those pages? File attached for reference ("Chapter 1" header begins on page 5 and should start on page 3). I'm also not able to flow the table of contents from page 2 onto page 3 which may be a related issue.

That's because your text frame on p. 2 is linked directly to p.4, not to p. 3.

Click on the blue arrow at the bottom right of p.2 and click inside the p.3 text frame to link them, then do the same to link p.3 to p.4.
