Not every time, but more often than not. Also, I always get following warnings EVERY TIME: Even though the warning appears, the file is updated normally (the warning appears for absolutely no reason, its just a nuisance) This has been going on since release of V2... If it is, for example, in a folder managed by OneDrive or another cloud service that can physically remove the file to the cloud to save space, leaving only a "stub" entry on the disk, it is possible that when Photo tries to read it to perform the update it doesn't get the complete file because the cloud service has already begun moving it.
