Welcome to Affinity Forums :)

You can drag the pdf over the opened photo in Affinity Photo. It will be placed as an embedded document. If the background of the written text appears white when placed in the photo document, double click it to open it in a new document tab then go to menu Document and tick Transparent Background. Return to your original document tab and check it the background becomes transparent.

If not it's because the handwritten text is saved as an image in the PDF. In this case, double-click the written text again to open it in a new document tab, select the written text layer in the Layers panel, then go to menu Filter ▸ Colours ▸ Erase White Paper, to remove the write background from the written text.

 

Alternatively, if you don't to embed the PDF in the photo document you can also open it independently as a new document using the menu File ▸ Open, selecting all of its contents/layers and copy/pasting to the document with the photo. You can use the Filter as described above to remove the write background from the written text if necessary.
