Publisher: how do I set up an A5 document in portrait orientation?

I want to create a document in A5 format and portrait orientation.

When setting up a document, the only orientation settings I can see are in 'New Document'->'Pages'->'Arrange'.
In that section I can either set the document to be vertical or horizonal.

However, neither of these options let me set the document to portrait orientation.

It is repeatedly shown that the way of setting the page orientation is not intuitive. Users are looking for orientation settings on the right side of the window.
