Looking at the "screenshot" (?!) of your settings, it's likely caused by profile mismatch because you are embedding the document profile into the PDF.

When you export are you exporting to PDF (For Print) or another PDF preset?

I have never mastered color management, period, so I cannot help with that.

Used your example doc (FOGRA39), exported using PDF For Print default preset, checked PDF using Packzview and PDF Studio (have no Acrobat Pro on Catalina).
All black text is K100.

I have found the error.

I always use FOGRA39 when I convert to CMYK.

The printer I sent the files to, did not use FOGRA39, when he opened the files.

The second printer I sent a sample file to, used FOGRA39 and then all text was in 0/0/0/100. So it was a human error!

Next time I send files to print, I want to point out that they need to use the right profile.

Or make sure not to embed the profile on export.
Or use PDF/X-4 if the printer accepts it.
