If you want this, use your ellipse as your main layer, then either drag the teeth, and tongue objects as a child layer under the oval lips. This will give you, your layered mask, or whatever you want to call it. You can get the same results by selecting the teeth and tongue, go to Edit-Cut, then select your oval and do a Edit-Past Inside. I selected the oval and modified the pressure settings for the stroke to make them look more like lips with a varying thickness. Sorry in advance if I didn't understand your request properly.
