Bookmarks and TOC missing from PDF?

When I open a PDF in Affinity Publisher, it doesn't preserve the hyperlinked table of contents or PDF bookmarks that were already in that PDF, and even if I try to put them back in manually with anchors, it makes them all the same level (there's no nesting). Additionally, I can't figure out how to view or edit the alt text for the images. How do I do this?

I don't think that is possible in APub.

I don't think any hyperlinks in the PDF survive when you Open it in Publisher.

You may need to recreate the TOC from scratch.
