How do I make a kawaii tongue like this?

As with most requests, try breaking down the design into shapes/lines that you can easily create within Affinity apps.

In the attached file, the black mouth shape is just an ellipse which has been converted to curves, slightly rotated, and had its top edge pulled down with the Node Tool. Select the pink ellipse, choose ‘Edit > Cut’ (which will leave the mouth shape selected) and then choose ‘Edit > Paste Inside’ to clip the cut ellipse to the mouth shape.

That looks fine to me, Josie, although I’m not sure why you bothered to group it.
