I do not profess to be a professional print artist just yet, so please bear with me!

I created a pattern piece at 72 dpi.  

I then created a pattern (written instructions) at 192 dpi.  When I inserted the pattern piece to create a single document, the pattern piece was skewed.  I changed the entire Document to 72 and that fixed the pattern piece and had no noticeable affect on the layout of the pattern instructions.

But I still want to produce a higher resolution PDF so my photos are crisper.  I exported at 192 and it looks nice.  

I'm just confused that when I changed the export PDF value to 192, that it did NOT revert the pattern piece back to the irregular shape.  I'm glad it didn't, but I'd like to understand better why.

I never experienced any object getting "skewed" by inserting within Affinity.
– What layer type(s) was the "pattern piece"? (pixel, curve, group, compound, ...)
– What method did you use to insert? (copy-paste, drag & drop, place command, place tool, ...)
– In what way got it skewed? (rotate, stretch, shear, ...)

If the inserted object had a stroke applied:
– Was the option "Scale with object" activated in the Stroke Panel?
– If yes: what if you untick this checkbox before inserting the object(s)?

Can you still reproduce this issue of an inserted object getting "skewed"?
– If yes, could you upload two documents, the one with "pattern piece" in 72 dpi + the other with "instructions" in 196 dpi ?

Thank you Thomaso for responding.  I have uploaded both files and here are the steps to reproduce "error."

1.  open ddhccover4 in publisher (192 dpi)
2.  Add pushpa big sis (AD Art board) file to above file (72 dpi)
3.  You'll see the corner in piece B is approx. is ~1/4" Square
4.  Change document dpi to 72 and the corner is now corrected to what it should be (1" square).
5. The cover page is not affected by this change visually.

It has nothing to do with adding one file to another. If you open the pattern page on its own and change its dpi, then the size of the cutout corner in the bottom left changes. What is slightly less noticeable is that the size of the rounded corner in the top left of the upper object also changes. 

Both of these corners were created as cusp nodes. I can't find much information about them in the help files, and I'm not familiar with the way they work. But one way to get round the issue is to select the Move tool, then select each curve and click Convert to Curves in the Context toolbar:

This redraws the objects with standard nodes, and you should now be able to change the dpi without changing the sizes of the corners (I wouldn't call it 'skewing' as that implies a slanting change of shape).
