I've got a table of contents I like. I get a preflight error for it saying, "one or more table of contents entries needs updating." When I click the "fix" button, it removes my formating, doesn't fix anything obvious, and the preflight error stays there like nothing happened. 

What's causing this and how do I fix it?

Off topic, the screenshot shows a TOC with several instances of “Of” with a capital O and several others where it’s lowercase. detective.gif

I don't have a solution for the preflight error remaining afte "fix".

However, as you mentioned "removes my formatting": that sounds like you are generating the TOC, then manually making changes to the text it contains. You can certainly do that, but if you do then whenever you regenerate or update the TOC you will lose your changes. Generally you should try to make changes in the TOC formatting by adjusting (editing) the TOC-related Text Styles, instead. Those changes will remain after the TOC is updated.
