With the Move Tool active, and the item selected, you should have a Replace Image button on the Context Toolbar: (This assumes you are using 1.8, and that you have an Image (PNG, JPG, TIFF). With Other file types or releases things may be different.) Yes, the 1.9 beta does adds two functions that may be relevant here: The ability to have Linked files, not just Embedded; and The ability to have them updated automatically if a Linked file is modified externally. But for Embedded images such as 1.8 supports, one needs to use the Replace button to get a changed file loaded into the document.
