Thank you for your offer, but I was able to correct the problem by deleting the page and recreating it with the master page. This was the only action required. I'd send the old file for review, but it is now deleted.

I don't know if we have had the same problem with the ToC but I found that deleting the Text Frame which contained the ToC is sufficient to fix whatever the problem is. I can't figure out what I am doing to get things messed up so I have not filed a bug report as I suspect it is me doing something wrong.

I have never mastered color management, period, so I cannot help with that.
