Publisher problem with inserting pages

Here's my problem:

I am preparing a four-page document. The double-page Master A has one title text field each.

Now I insert a new page with Master A before page 1. But now title 1 does not move to page 2 but disappears. Sometimes also the titles are moved over each other, for example title 2 over title 3. See screenshots.

What am I doing wrong? Or is this a bug? Thanks for a hint.

If you have a ‘facing pages’ Master A, why are you inserting only one page instead of two?

I want to insert a table of content of only one page. Isn't it possible to insert only 1 page?

If you have double-page spreads you’re always going to have two pages per spread (i.e. four pages per sheet). The back of a table of contents page is usually blank, but you have to add that blank page yourself instead of leaving the software to guess that this is what you want.
