A few things that would be good to know, what Affinity program is this? Are you on Mac or PC? Which OS version are you using?

Does this happen only when you try and place the same image or any image?  If you open up the application and do not try and place an image can you make a new document and proceed as normal?

¿Esto sucede solo cuando intentas colocar la misma imagen o cualquier otra imagen? Si abres la aplicación y no intentas colocar una imagen, ¿puedes hacer un nuevo documento y proceder normalmente?

I use affinity photo 1.8, my operating system is windows 10 pro, the shutdown happens when placing any image and it also happens when I use any tool.
