Header and footer not using master format

The headers and footers are misbehaving.

You can see on page XIII that it is ignoring the style set out in the master. 

Also on XIII you can see the red dots on the header - suggesting there is other text there. I do not understand why.

The style issue is also happening - seemingly at random - to the headers on the following two pages. 

I have tried reapplying the master without success.

When I reapply it, the text flow in the master is also missing.

The problems seem to have started since I attempted to insert a table of contents.

I've attached the file - hopefully someone can set me straight.

There is more text in there, "Foreword by Barbara Lamb Foreword by Andrea" is what is in the frame.

As to the styles issues I see that you have the Group style Base applied to the text in the headers instead of an actual Paragraph Style.

I have never mastered color management, period, so I cannot help with that.
