Transparency problem with image inside PDF made with Photoshop

A customer has supplied a Photoshop created PDF containing an image which was given a transparency within Photoshop. When I open the PDF in APub the image is still behaving as if it has a transparency applied. If I rasterise the image to turn it into a pixel layer the same transparency thing happens. I can see a red circle placed behind the image. The image has no opacity applied within APub. Is this expected behaviour and is there any way to remove the transparency of the image?

Another problem with this file. I have flattened the image in Photoshop and relinked the image. It's better (I think) but now when I try to open the file the program immediately closes.
