As expected, page size is 8.5x11 inches: Change something, perhaps the width and/or height to 9x12. As expected, the layout panel in the dialog now shows Custom rather than Letter. However, the Letter preset is still highlighted: Now, click on either Create or Cancel. Then use File > New again. The dialog looks just like the above, with Letter highlighted, but Custom in the Layout panel and the changed values. Not wanting to have to reset the changed values, one might try clicking on Letter again. Nothing happens. Or, one might double-click on Letter. Doing so creates a new document with the Custom values shown in the Layout panel, not the size shown under the Letter icon. One must either change the values manually to get them back, or (simpler, once one knows) click on a different preset, then click on Letter again.
