How to get a selection to apply

This may seem like a dumb question, but how do you get a selection to apply.  I draw a path around the area, refine it, but I still end up with the whole image rather than the part of the image that I selected.  When I check options it seems to imply that a selection has applied. 

I have had this program for years now, and I think it's fantastic, but this bit seems to elude me.  I have tried everything, watched tutorial after tutorial and gotten nowhere.  Some tutorials have suggested solutions, but tried it with the same result.  So what on earth am I missing here?  I'm sure it's something ridiculously simple, and probably staring me right in the face, but it's got me beat.  What is the next step from drawing a line or a shape etc around what you want to select?  Every single tutorial I have encountered seems to imply that the selection is automatic and the whole thing should just happen by magic, but that isn't happening for me
