Drawing a brain ... or pipework ... or spaghetti

I'm not sure that this is even possible, but I'd like to be able to use affinity designer to draw a human brain, pipework or spaghetti - anything really with that kind of structure. I've tried using the pen tool and applying a 3D effect but the whole shape just flattens.

I'm after being about to reproduce the red stringy bit in this image ...

Add an outer shadow fx afterwards because it will be applied to the curves and should shade it as you would expect. but nice squiggly brain brush lol!

It only applies the shadow to the shape as a whole rather than the curve ...

Draw the curves in stages so they are layered
