I'd love to see a test document for this issue because I wasn't able to duplicate it when it was previously reported.

Check all of your TOC and Index style definitions, especially which styles each is based on. I tried doing silly things like basing Heading 1 on Index Entry 1, and Index Entry 1 on Heading 1, and TOC 1: Heading 1 on Index Entry, but I couldn't find any way to trigger the issue.

Also, is your TOC in its own text frame(s) with no other text? Is your Index in its own text frame(s) with no other text? I consider those best practices to avoid issues. Unlike other apps, Affinity provides no indication of where the special TOC and Index content starts and ends and it's easy to mess it up.

If you're having the issue with an index appearing in the table of contents, please review this post and ensure that you haven't accidentally included an index style in the table of contents.
