Noise reduction and Hot pixel reduction is different functions. Of course, it depends on the camera whether and how it is implemented, but the hot pixel reduction is as you described in the first post (two exposures under the same conditions - same setting and same time), but the reduction is done before the actual image data is processed. Doing it after processing images will not produce such results.
