New is highlighted on the left. There's a Create button on the bottom. 
You might watch some of the Tutorials, or the What's New in V2 section of the application Help (in the app, or online at https://affinity.help both of which will give some ideas about dealing with the new way things are done in V2.

Make your choices and see what happens.

It's the new document window. So choose any of the items from the middle column & click "Create" or change any of the settings if you want.

In my copy of the workbook, the Smudge section begins on page 394. On the prior page (393 for me) the author of the article explains that he created the mask layer for later use but realized that it was not needed so he flattened the image to a single layer using Layer > Rasterize. 
This is consistent with what the Layers panel picture shows on my page 394, which is what the panel should look like after step 1 on that page is completed (renaming & duplicating), resulting in just two layers.

This is working as expected for myself.  How many layers have you got at this point?  You should have two, one called Smudge and one called Disintegrate.  It sounds like you might have these layers the wrong way round.  Could really do with seeing a screenshot, try resizing your screenshots so they are no wider than 2000 pixels and try uploading them again

Edit > Fill with Primary Color did not exist in AP version 1.6 this was added later in AP version 1.7
Version 1.6 only had the Edit > Fill menu command
The book (I believe) was originally written for version 1.6

Hi PaulD,
1. Both function the same however duplicate will duplicate only objects that have been selected where as duplicate selection will duplicate everything inside your marquee.
2. Merge visible merges all layers into a new pixel layer.
3. Rasterise Mask rasterises an object then converts it into a mask.
4. Edit > Fill allows you to give an object a fill. Edit > Fill primary colour will fill the object with your primary colour stop.

Yes.  When I selected the cheese image I included the background above the cheese, mistakenly. That explains Question #2. 
I have trouble painting in masks, so I need to watch the process more carefully. I believe you are missing the detail for this.

Got it. By clip, you mean drag under A-text. It worked but I had to re-color text.

Thanks... got it!

It could be just pressing tab which erases toolbars and panels.  Then tab shows them again.
It could be shift cmd H which hides just the studio of panels, menu View>Studio>Hide Studio or shift cmd H again. 
Do either of those work for you?

'Edit With' feature solved problem. Never use extensions?

What happens if you use the 'Edit With' feature instead of the Affinity Photo Extension?  This can be accessed by right clicking the image when viewed in Apple Photos.
