:D Walt, you've saved my day.
I had the problem of none existing Heading1 showing up in the TOC.
Having a good look at the layers panel solved all weird problems.

The order of entries in the TOC, when both items are on the same Spread, is determined by their order in the Layers panel, from bottom (first added) to top (last added).
It sounds like you created the Text Frame for "Introduction" after the one for "Test subtitle", which makes "Introduction" logically (in layer order) below/after the subtitle.
Just drag it lower in the layer stack and then update the TOC.

Welcome to Affinity Forums :)
The next Affinity Designer beta (1.5) will include Symbols which will lets you use several instances of an object in your project. Updating one will update all instances at once.
There's a few more options that makes this more powerful. MattP (one of the developers) post a sneak peak of the Symbols feature here. More details on his post here.
