[table of contents update deletes entries]

All the style boxes were checked. The problem was that the flow option was set to move each entry onto a new page. I did not realize that option was selected, evidently form an earlier edit, and so the new table only showed one entry for the page.

The TOC update deletes all my entries except the first two or three. I have it set for the entire document and the correct styles checked, but when I click update it deletes all of them except the first two or three. I have tried undoing it and recreating the entire table of contents, but it will only now show the first few entries. Has anyone had this problem? I have undone and redone it several times and cannot get a complete update of the TOC.
