I have not (as yet) had that problem on my Mac, but it does sometimes happen on my iPad -- I get the "Loading" message with the spinner but the file does not appear anywhere in the document, nor in the Layers Studio. Seems to happen about 50% of the time, but I can't pin it down to anything like the number of vector objects in the file, its dimensions, or anything else.

We are talking about the beta version here, not the retail. The current retail does not show this misbehavior.

For a very small puzzle, I proceed as follows: after creating the pieces individually and assigning them a slight 3D fx, I place a copy of the photo inside each of them.
It's then possible to move one of the pieces.
If I want a ghost image of the moved piece, I duplicate it before moving it, remove the 3D fx and reduce its opacity.
For the background, I place a very light grey square under the pieces.
I've attached a very simple example of four pieces, with and without ghost image.
I'm well aware that this method can't be used for large puzzles, as it requires a lot of manipulation, but I'm just adding my voice to the discussion.

In the layers, you can nest the flag in the rectangle thumbnail.

Click on the red button to access the file.
I exported them from v1 and installed them in v2 without any problem, I hope this helps.

There is a Plugin that is compatible with Affinity called Akvis Artefact Remover: https://akvis.com/en/download.php and while it doesn't always remove all of the artefacting it can certainly speed up the process and help make the tedious removal, less so.
It's also free ,although an email registration is required to get a licence key.

Here is the requested video. As @NathanC says, I noticed that the tool works better using individual clicks instead of dragging it across the drawing.

@WelpITried, type the text, apply an "Outer Shadow" fx to it. Set "Fill Opacity" to 0% in the "Layer Effects" tab.

Try just using Ctrl+Y again.
Ctrl+Z would not undo it, because it's not a change to the document.

I tried ctrl+z to undo the redo, but it didn't undo all the lines.
I finally just gave up.

Wireframe - thank you. I didn't see an option for that, but I was kind of pissed at that point I may have just missed it.

After redoing "Ctrl+Y" as suggested by @walt.farrell, (or unchecking the "Wireframe view mode" button), you should be able to go back in your project by doing "Ctrl+Shift+Z" which is the new shortcut to undo the effect of "Ctrl+Z".
I'm also confused about this shortcut change.

Create a new complete Ellipse that matches the outermost edge of the blue shape.
Duplicate the left T and E, and Boolean Add them to the Ellipse to make a new shape.
Apply Contour Tool to the new shape, and give it no fill and an appropriate stroke.
