I'm having the issue too of the adjustment layer not extending to the bleed area but I don't have any layers grouped. I'm trying to add a layer to take my color version and change it to black and white. ... Ah ha! I just zoomed in to get a close up screen shot and it redrew the image. It looks like my issue was only that the bleed area wasn't redrawn after creating the layer. Just by toggling whether or not it is visible replicates the behavior. So, sorry _TM, my issue doesn't seem related to yours.

I would like to say thanks to >|< for solving the issue for me. I don't understand why it behaves this way, but it is infuriating! The Extend option doesn't show unless in a vector object. I'm attaching a screenshot hoping it might help someone.
